Personal Care
Our team comprised of nurses, MNAs LNAs, and caregivers will meet residents where they are and will assist them with activities of daily living including personal hygiene (bathing, grooming, oral care), dressing, eating, toileting, and transfers.
Medication Management
Our nursing staff will provide oversight of all medications prescribed to our residents. They will coordinate care with our in-house Nurse Practitioner, outside medical doctors, and third party services providing physical therapy, occupational therapy, and speech therapy. Our nurses will manage the ordering, receiving, preparation, and storage of medications so that you or your loved one receives the care they need without the worry of having to manage medications.
Memory Care at Harmony Homes By the Bay
Our Memory Care Program offers specialized supervision and a secured environment for residents with Alzheimer’s and other forms of dementia. Our facility and activities program is designed to provide a familiar and supportive atmosphere for our residents to fulfill their potential and succeed.
As a convenience to our residents, we have a traveling nurse practitioner that will see and evaluate residents as needed without having to coordinate rides to and from doctor’s offices. This allows the resident to be assessed faster and to be seen in the comforts of their own home. Residents can see our in-house nurse practitioner, or choose to stay with their primary care physician.
